The latest version of pandoc (1.16.x) shipped in Ubuntu 16.04 is more
restrictive on markdown syntax so we have fixed it.

Previous versions (4.0-4.1) incurred several errors when estimating the fiber length.

By definition, the speed v at which light travels through a medium with refractive index n is v=c/n
Since for the ITU-T G.652 D recommendation:

	"the refractive index profile of the fibre does not generally need to be known"

we chose a value provided by one of the most popular G.652 fiber manufacturers
for 1550 nm in their product information as seen in:

Changes:
- The Fiber calibration table has been removed due to the value vas never saved because the upper table was always being referenced
- endpointcalibrationalpha.php contains now the fiber calibration
- The header of the endpoint calibration table has been updated to match the measurement requirements
- menu.php updated with new links and reordered

This is usefull to check that the calibration value are correctly
configured when calibrating the web interface.

When many (10 or more) non-wr compatible SFPs were used then could happen that
snmpd was not able to start (sometimes even crashing).
The problem appeared only when the logs from snmpd were not redirected,
so it too long to print the logs to the slow serial console.
Before logs were printed, the content of HAL's shmem changed and this caused
to re-read HAL's shmem and print logs again and again...

The fix is to print logs after the shmem is read.
