- corrected small errors (readreg instead of writereg in "Remote reset"
  section, various typos
- added a little bit of info
- made "Revision history", "List of abbreviations" and "References"
  sections appear in the Contents and the bookmarks of the .pdf
- also made Contents appear in bookmarks, without it appearing in the
  actual Contents

The modifications can be read from the .pdf doc, Revision history
section. First `make' the ug, then go to this section to see the
modifications

Also added channel frequency in output file.

This .gitignore will avoid headaches when changing your ei2cdefine.py to
contain username and passwords for your ELMA crate.

The old gwvers.py script has been changed to getsr.py, which
lists the fields of the board status register in a more comprehensive
manner.

Then, the setsr.py script has been added as a means to clear set error
bits.

Note that unlike what the git commit output below states, the
gwvers.py was actually renamed (and changed to) getsr.py.

The script was reporting a success in writing the .mcs file even though
PROMGEN exits with an error and an .mcs file is not generated. This
error is now fixed by checking the return value of the PROMGEN command.

The most major update is the fact that the login is not manual any more.
Instead, a common file `ei2cdefine.py' is used to define the ELMA crate
hostname, username and password. This module is used across all Python
scripts to connect to the ELMA crate.

Apart from that, some new modules have been added and the gp-py/ folder
was renamed to diag/

The readflash.py tool can be used to read the flash from address A
to address B, where A and B are numbers selectable by the user. Checkout
README.txt for more details.

In multiboot.py, I updated the gateware version for the pulsetest
gateware (was wrong value of 0x99 before).
