  - it adds a new header for sysfs
  - it changes zio-sys.c to support per-instance trig/buf attributes

general: input and output is always cset-based · 41d776d4

This changes some method and stuff, in order to make all input and
output be cset-based. This solves a number of issues with the previous
setup. Thus, even drivers must be made differentlym abd this fixes
zio-zero accordingly.

As a side effect, we have a data_done method in trigger ops, so the device
can tell when it is done. And the fire_trigger returns void, since it
cannot wait for stuff to be done. See documentation for details.

zio invokes try_module_get() on buffer, trigger and device owners when
they are in use. For devices we still have a little unsafe time.

To be more suitable for different situation, the returned value is
changed from zio_obj_head to zio_object_list_item, which contains the head
and other useful information.

zbuf_find_by_name() and trig_find_by_name() are modified too,
according to __find_by_name()
