Fix to CDC constraints generator matching filters

The main focus of this MR is to fix the regex matching filter of the generate_cdc_constraints.tcl script to allow the synchronizers to have the automatically generated prefixes.

In this MR I also reinstated the original constraint files for synchronizers. Albeit the usefulness of the constraint generator script, the traditional module-based constraints are still be useful for certain use-cases.
